Overview

Based on the current version of the App, this Privacy Policy describes how Lone Star Family Care handles information in connection with the Lone Star Family Care iOS application (the “App”). Lone Star Family Care does not collect personal information through the App, does not require users to create an account, and does not store user-entered health data on Lone Star Family Care servers.

Information the App Does Not Collect

Based on the current version of the App, the App does not itself:

• Require account creation or login
• Collect your name, email address, phone number, or similar identifiers through the App
• Track your precise location
• Use analytics, advertising SDKs, or third-party tracking technologies within the App
• Store user-entered health data on Lone Star Family Care servers
• Sell personal information

Notification Permissions

The App may request permission to send local notifications to deliver medication reminders you configure. These notifications are scheduled and delivered entirely on your device. No notification content or scheduling data is transmitted to Lone Star Family Care.

Information Stored Locally on Your Device

The App includes optional tools that allow you to enter information for personal reference. Information entered into these tools is stored locally on your device only using standard iOS local storage. Lone Star Family Care does not receive this information through the App.

• Medication Reminder & Log: Medication names, dosages, frequencies, reminder times, and taken history may be stored locally on your device.
• Symptom Journal: Symptom entries including name, severity, date, body area, duration, triggers, medications taken, and notes may be stored locally on your device.
• Previsit Notes: Visit notes including reason for visit, symptoms, questions, medications to discuss, and goals may be stored locally on your device.
• Personal Health Data: Any optional personal details you choose to enter, such as name, age, blood type, allergies, or emergency contact, may be stored locally on your device. This information is not transmitted to Lone Star Family Care or any third party through this App.
• Blood Pressure Log: Readings such as systolic, diastolic, pulse, date, and notes may be stored locally on your device.
• Hydration Tracker: Daily glass count, goal, and recent history may be stored locally on your device.
• Adult BMI Calculator: Calculations are performed within the App and are not transmitted.

You can remove locally stored information by clearing it within the App where that option is available, or by deleting the App from your device. Device backups and device-level settings are controlled by Apple and your personal device configuration, not by Lone Star Family Care.

Third-Party Services and External Links

The App may provide links to, or open, third-party services. When you leave the App or open an external service, that third party’s terms, policies, and privacy practices apply. Lone Star Family Care is not responsible for the privacy practices of third-party services.

• Healow Scheduling and Patient Portal: The App may open Healow pages in Apple’s built-in browser. Use of those services is governed by healow, LLC’s terms and privacy practices, including the Healow Privacy Policy.
• Apple Maps: Directions may open in Apple Maps. Use of Apple Maps is subject to Apple’s privacy practices. Apple Privacy Policy
• Phone and Mail: Tapping phone or email links may open your device’s native apps, subject to Apple’s and your device provider’s settings and policies.
• Web Browser and Social Media: Website, Instagram, TikTok, and similar links may open in Safari or another browser, where those third-party services’ privacy practices apply.

Medical Sources and Reference Links

The App may provide links to external medical reference materials for general informational purposes, such as blood pressure and BMI guidance. These reference pages are provided by third parties and are subject to their own terms, policies, and privacy practices.

The Adult BMI Calculator, Hydration Tracker, Blood Pressure Log, Medication Reminder & Log, Symptom Journal, Previsit Notes, and all related health content in the App are provided for general informational and personal tracking purposes only. They are not medical devices, are not intended to diagnose, treat, cure, or prevent any disease or condition, and do not replace professional medical advice, diagnosis, or treatment.

Children’s Privacy

The App is intended for general patient and caregiver use. Lone Star Family Care does not knowingly collect personal information from children through the App.

Data Security

Information entered into the App’s optional local tools is stored on your device. The security of that information depends in part on the security of your device, operating system, passcode, backup settings, and other device-level protections. While Lone Star Family Care aims to minimize data handling within the App, no method of electronic storage can be guaranteed to be completely secure.

Changes to This Policy

Lone Star Family Care may update this Privacy Policy from time to time. Any changes will be reflected by updating the “Last Updated” date above. Your continued use of the App after an updated Privacy Policy is posted indicates your acceptance of the revised policy.
